Frankly, my first thought would be a defective keyboard/stuck key. Especially since it's the last thing changed before the problem appeared. I'll ask my mac expert if she has any thoughts.

Sounds like WoW/keyboard or WoW/driver bug, since he never sees it unless he plays WoW. I'd
* do all the debugging he's already done, (Check)
* play WoW with a different (even if less awesome, in fact preferably generic) keyboard long enough to see if it is the keyboard/fancy driver. If he can limp along with only his quasi-sentient mouse, presumably he can limp better with a plain-Jane keyboard for in game IM, long enough to test things.
* return it for a new Deathstalker.
* find out from his support requests that the Masskiller III (or equivalent) is actually less buggy with WoW
There are one or two third-party mouse/keyboard extra functionality drivers out there for Mac that _might_ give him extra capability with a generic keyboard. But I suspect he's much better off finding the Deathray XII that does what he most craves, since he is facile with these things.
I updated the post to say I also did a rename/delete of the WTF, Interface and Cache folders with the same results.
I'm worried that is the case, H. I like the keyboard though. For now I think I'm going to remap my Ctrl modified hotkeys to be Alt instead. Might not be permanent but at least it's something to test.
Seems that it was a corrupt .kext file and not the hardware. I manually deleted all of the files and reinstalled the software. This completely cleared up the issue.
